Idaho Medical Massage verifies insurance benefits before scheduling appointments when insurance will be used for payment.
If you would like to use your insurance benefits, please complete the Insurance Verification Form using the link below.
As a private, one-provider practice, I personally manage insurance verification and patient follow-up. Due to the volume of referrals and verification requests I receive, please allow up to one week for benefits to be reviewed.
Once your benefits have been reviewed, I will contact you to discuss your coverage, answer any questions, and review your scheduling options.
Please note: Insurance verification is not a guarantee of payment or coverage. Benefits are determined by your insurance plan, and you are ultimately responsible for understanding your coverage and any applicable patient responsibility.
