Privacy Policy
We are committed to our client’s privacy. We collect, use, and disclose your information responsibly. At Idaho Medical Massage we are aware of the sensitive nature of the information you disclose with us and are dedicated to taking measures to protect your privacy and the regulations that are required to do that. Our privacy protocols are HIPAA compliant. Your Personal Identifiable Information (PII) and Personal Health Information (PHI) are as important to us as our quality of care for you.
At Idaho Medical Massage we ensure
• Only collecting necessary PII and PHI
• Only sharing information with consent
• Storing, retaining, and disposal of your PHI, PII complying with existing legislation and privacy protection protocols
How we Collect, Use, and Disclose Patient’s Personal Information
• To deliver safe and effective client care
• Communicating with other health care providers
• To communicate with health insurance, lawyers, and other supports persons as required to provide service to you (when applicable)
• To maintain communication with you
• To advise you of treatment options and referrals
• To contact you, schedule or confirm appointments and distribute information about the clinic
• To invoice you for services
• To process credit card payments (when applicable)
• To collect unpaid accounts
• To comply with the office’s regulatory requirements
Our office does not supply your confidential medical information to any third party without your written consent. You may withdraw your consent for use at any time.
Cancellation/Rescheduling/No Show Policy
Cancelling an Appointment
Please contact Idaho Medical Massage via phone AT LEAST 24 hours prior to your scheduled appointment date and time to avoid cancellation fees*.
*Cancellation is required 24 hours prior to your scheduled appointment; failure to cancel within the required time will result in 100% charge for your scheduled service to the credit card on file. A no show is considered a failure to cancel or show up to a scheduled appointment and will result in a 100% charge for your missed service and will be applied to the card on file.
Deposits
A $50 dollar deposit fee is required for all scheduled appointments. The deposit and is put toward your total service charge when paying the remaining balance for your service. The deposit is fully refunded to the client if the appointment is cancelled within the 24-hour cancellation notice.
Clients on Treatment Plans
Clients on treatment plans who have no shows or late cancellations for their scheduled appointments will be automatically deducted the cancellation fee from the treatment plan balance.
We reserve the right to refuse appointments to any client who has demonstrated disregards of our cancellation/rescheduling policy.

Late Arrival Policy
We understand that life can be unpredictable. However, our Late Arrival Policy helps maintain a quality experience for all clients, respecting both your time and ours.
Arrivals beyond the 5-minute will result in a reduction of hands-on time during the session to avoid delays for subsequent clients.
The full fee for the originally scheduled service will be charged, regardless of reduced time due to late arrival.
Full Payment: Rescheduling: If you're running more than 15 minutes late, we will reschedule your appointment and will be charged full price for the appointment.
If you know you’re running late, please call us as soon as possible. This allows us to make real-time adjustments and informs your therapist.
Clients who consistently arrive late may be asked to pre-pay for future services or may no longer be serviced.
By scheduling an appointment with us, you acknowledge and accept these terms. Your cooperation allows us to better serve all of our clients and is greatly appreciated.
GRATUITY POLICY
We appreciate the thought but we do not accept any tips. Thank You!
Idaho Medical Massage Policy Statement & Waiver

If I experience pain or discomfort during the session, I will immediately inform my therapist so that pressure/strokes can be adjusted to my level of comfort. I understand that to receive the best possible massage that my communication is necessary and expected and will not hold the therapist responsible for any pain or discomfort should I neglect to voice my preferences and concerns.

I understand that the services offered today are not a substitute for medical care.

I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.

I affirm that I have notified my therapist of all known medical conditions and injuries.

I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so.

I understand that draping will be always used for the comfort of both the client and the therapist. I understand that if I become uncomfortable for any reason that I may ask the Therapist to end the massage session, and they will end the
session.

I understand that massage is entirely therapeutic and non-sexual in nature and that the massage therapist may end the session for any inappropriate behavior. I understand that I will be financially responsible for the entire session and may not
be allowed to schedule any future sessions.

By signing this release, I hereby waive and release Idaho Medical Massage from all liability, past, present, and future relating to massage therapy and bodywork.
